Money Follows the Person Demonstration Project

 

• What is the Money Follows the Person Demonstration Project (MFP)?

The Money Follows the Person Demonstration Project affords individuals who are institutionalized the option of returning to the community and provides supportive services necessary to maintain them safely in the comforts of their own home. MFP is one part of a larger effort to rebalance the long-term care system in the state of Connecticut.

• Who is eligible for MFP?

To qualify for the Money Follows the Person Demonstration Project, the consumer must be eligible or actively receiving Medicaid benefits and be have resided in an institutional setting for three months or longer.

• What are the benefits of MFP? Below is a list identifying some of the benefits of the Money Follows the Person Demonstration Project. Please note, the list below is not all inclusive and not every consumer will be eligible for the same benefits:

1. Access to housing coordination
2. Emergency 24 hour back-up system
3. Special Funds to assist with purchasing items necessary to relocate to community living.
4. Transition services for 6 months
5. Rental Assistance 6. Access to Medicaid Waivers for supportive services (if eligible)

• Who are transition coordinators and what is their role?

Transition coordinators are charged with the responsibility of assisting you in your transition to the community. They will assist with linking you to the proper community resources, collaborating with you as well as the nursing facility staff, monitoring your services, and ensure you have a safe and effective plan for returning to the community.
