The VDHCBS program serves Veterans of any age who are at risk of nursing home placement and their family caregivers. The VDHCBS program provides Veterans the opportunity to receive home and community based services that enable them to avoid institutionalization and continue to live in their homes and communities.

The VA Health Care System identifies veterans who may be eligible for the program.

• How do I refer myself or my loved one to the VDHCBS program?
The VA Medical Center will identify Veterans who are eligible for the VDHCBS program and refer to AASCC. If you have not been referred to this program and would like to make a referral, please contact your social worker at the VA Medical Center.
