 AGENCY ON AGING PROGRAMS 

The Agency on Aging's many programs include (CLICK on the links for more information):

Aging Resource Center

Community Choices/Aging and Disability Resource Center (ADRC), provides comprehensive information on the wide range of available private and public long term care options, programs and services. Also conducts assessment of individuals’ long-term care needs

CHOICES is Connecticut’s program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening.

SMPwhich addresses issues of health care fraud, errors, abuse and other related health care scams.

Care Management Department

Connecticut Home Care Program for the Elderly (CHCPE) provides assessment of need, ongoing care management and subsidized home care services to eligible individuals age 65 and older, as well as supporting their caregivers.

Connecticut Homecare Program for Disabled Adults (CHCPDA) provides care management and services for individuals 18-64 who have a diagnosis of a degenerative neurological condition and need supportive services. 

Respite Care Management Programs

     National Family Caregiver Support Program provides services for caregivers of adults age 60 and over needing assistance due to a medical disability.

CT Statewide Respite Care Program assists caregivers in developing a respite care plan for their family member diagnosed with dementia. 

Bridge Program provides short-term assistance for individuals age 60 and over, with no family support, whose medical disability has limited their independence.

Money Follows the Person affords individuals who are institutionalized the option of returning to the community and provides supportive services necessary to maintain them safely in the comforts of their own home. 

Veterans Directed Home and Community Based Services Program (VDHCBS) serves Veterans of any age who are at risk of nursing home placement and their family caregivers.  The VDHCBS program provides Veterans the opportunity to receive home and community based services that enable them to avoid institutionalization and continue to live in their homes and communities.

Volunteer and Training Department

Senior Service Corp is a federal volunteer initiative for individuals 55 and over. Foster Grandparents Programvolunteers work with preschool age children and Senior Companions Program volunteers work with their frail peers (stipend available).  RSVP volunteers offer their skills, experience and talents to local community organizations, helping to strengthen and improve their communities.

Experience Corps trains and places volunteers age 55 and over in schools for 12-20 hours per week as tutors for students in grades K-3 (some positions have a stipend available). 

VISTA places volunteers in community organizations which are working to improve academic success in the city of New Haven.

Opportunities for Older Adults  individuals are assigned to work with older adults with developmental disabilities (stipend and paid positions).

Senior Community Employment Program (Title V) provides paid job training and assistance in job placement.

Grants and Communications Department

Older Americans Act grants oversight, under which the Agency on Aging grants staff are charged with evaluating applications for and monitoring ongoing use of Older Americans Act funds that support in-home and community services (targeted toward persons age 60 and older who reside in the south central region of Connecticut).  Programs funded include adult day care, transportation, legal services, chore/homemaking services, health screening/promotion and home delivered and congregate meal

Caregiving Resources - Cash & Counseling Training and Video
